Accessibility
Open Door Access recommends environmental modifications for “visitable” private homes; addresses universal design for public buildings, new and renovated construction, remodels, and renovations. Recommendations address the interior and exterior of a location, including appliances, equipment, lighting and durable medical equipment.
Geriatric Care Management
Open Door Access works on behalf of older adults (together with their families and friends) to preserve their self-determination in daily life, including:
- being independent at home as long as desired
- being connected to community resources
Education and Community Outreach
Open Door Access provides education and community outreach to promote knowledge and inclusion. We offer presentations regarding accessibility and care management, including standards, legislation, equipment, community supports and resources.
What we do
At Open Door Access, we work with you to identify your priorities and goals. We engage in a comprehensive process of analysis and resolution in four basic stages; assessing, planning, implementing and evaluating the outcome.
Assessing: In determining your priorities and goals, we use standardized assessments, interviews, and observations to identify strengths, weaknesses, opportunities and threats. This may include, at your request, consultation with significant people in your life. Our areas of assessment may include:
- your home, work and play environments
- your personal care routines such as eating or bathing
- your daily living tasks such as cooking or home maintenance
- your community, including family, friends and other supports
Planning: We use the results of the assessment as the basis of a plan which includes short and long-term goals. The plan establishes your priorities for daily living. We then offer a variety of strategies to achieve your goals, according to your preferences and situation.
Implementing the plan: We focus on solutions designed to support your independence and connection to the community. In implementing the plan, we may suggest options such as:
- making your environment safer and more accessible
- teaching you new techniques or routines of daily living
- recommending equipment for independence in your personal care or daily routines
- identifying existing community resources and services useful to you
Evaluating outcomes: Together, we will determine the extent to which your goals have been met based on your personal standard. We evaluate outcomes considering the degree of effectiveness of the solution, the efficiency and the satisfaction you experience.
